Position Overview As an RN Case Manager in Minnesota, enjoy true autonomy while making a profound difference. Visit patients one-on-one in their familiar home environments, conducting comprehensive head-to-toe assessments, coordinating care, managing diseases and medications, performing skilled wound care, catheter/ostomy management, infection prevention, and home safety evaluations. Work collaboratively with interdisciplinary teams to optimize outcomes in the southeast Twin Cities Metro territory.
Competitive Compensation & Benefits - Annual Salary Range: $89,419 - $106,620

Schedule: Full-time days, Monday-Friday with holiday rotation (about 2/year); weekend rotation may apply based on staffing.

Key Responsibilities - Perform assessments, procedures, and skilled interventions requiring clinical judgment across psychological, biological, physical, and social sciences
- Evaluate care quality, review clinical notes, and participate in ongoing interdisciplinary patient assessments
- Execute physician-ordered services per individualized care plans
- Educate patients/families on healthcare needs, self-management, and safety
- Document progress accurately and communicate effectively with healthcare teams for seamless coordination and discharge planning
Qualifications - Graduate of accredited nursing school with current MN RN license
- Reliable transportation, valid driver's license, auto insurance
- CPR certification required
- Preferred: 2+ years nursing experience; 1+ year case management; community health/Med-Surg background
