Ambulatory Clinical Pharmacist at Providence Health and Services summary:

The Ambulatory Clinical Pharmacist collaborates with healthcare professionals to deliver comprehensive patient care in an outpatient setting, ensuring safe and effective medication use. The Senior Clinical Pharmacist will collaborate with physicians and staff to provide coordinated, comprehensive, high quality care to patients.

Required Qualifications:

  • Bachelor's Degree: PHARMACY from an accredited School of Pharmacy., Or
  • PharmD: PHARMACY from an accredited School of Pharmacy.
  • Coursework/Training: Postgraduate Year One (PGY-1) Pharmacy Residency, Or equivalent education/experience
  • Upon hire: Washington Pharmacist License
  • Within 2 years of hire: Certification from Board of Pharmacy Specialties
  • Upon hire: Board of Pharmacy Preceptor License (Facility Managed) - when eligible and where applicable
  • 3 years - Pharmacy practice experience with at least 2 years of related experience in lieu of Postgraduate Year One (PGY-1) Pharmacy Residency.
